Original form, approximately A4 size, signed on February 5, 1941. It is a questionnaire completed by a police officer, in which—in accordance with the regulations of the occupying forces—the complete family tree of himself and his partner was provided. The purpose was to demonstrate that none of the named parents or grandparents belonged to the Jewish faith.
These types of documents mark the systematic registration and exclusion of Jews in the Netherlands, and demonstrate how the civil service was put at the service of occupation policy from the early war years onwards.
